Therapists face unique credentialing challenges: commercial payers use separate Managed Behavioral Health Organizations (MBHOs) for mental health benefits, Medicare expanded eligibility in 2024, and each license type has different taxonomy codes. MBHO Credentialing Is Required for All Therapists

Every major commercial payer uses a separate MBHO: UHC uses Optum, Cigna uses Evernorth, BCBS plans often use Beacon/Carelon. Credentialing with the commercial plan alone is not sufficient — therapists must credential with the MBHO to bill for therapy services. ProEnrollment submits to all MBHOs simultaneously with commercial plans.

Medicare 2024 Expansion

LPCs and LMFTs became eligible for Medicare enrollment effective January 1, 2024. LCSWs have been Medicare-eligible since 1989. If you are an LPC or LMFT who has not yet enrolled in Medicare, this is a significant missed revenue opportunity. See: Medicare enrollment services.

The MBHO Map Every Therapist Needs

Commit this to memory: UnitedHealthcare routes therapy through Optum Behavioral Health. Cigna routes through Evernorth. Many BCBS plans route through Carelon (formerly Beacon Health Options). Various regional plans and Medicaid programs route through Magellan. Aetna handles behavioral health largely in-house. Being credentialed with the medical plan without its MBHO means therapy claims deny — and the denial codes won't explain why. Apply to both layers simultaneously, always.

License-Specific Notes for 2026

LCSWs: Medicare-eligible since 1989, taxonomy 1041C0700X, broadest panel access. LPCs and LMFTs: Medicare-eligible since January 2024 under new MFT/MHC categories — enroll now while the provider population is still catching up and Medicare caseloads are wide open (taxonomies 101YP2500X and 106H00000X). Pre-licensed associates: most MBHOs won't credential them; group practices building on associate billing need payer-by-payer verification of supervision rules before hiring.
