Step 1: Benchmark Your Current Rates
Pull your contracted rates for your top 20-30 CPT codes by volume and revenue. Compare against Medicare fee schedule percentages and regional commercial data for your specialty to identify your most undercompensated codes.
Step 2: Document Your Practice Value
Quantify your network contribution: patient panel size, annual visit volume, quality metrics, geographic coverage, and any network adequacy gaps in your specialty and region.
Step 3: Reach the Right Contact
Rate negotiations go through the payer's Provider Contracting department — not credentialing, not provider relations. Submit a written rate request specifying exact CPT codes and target increases.
Step 4: Negotiate the Counteroffer
Payers rarely accept the first request in full. Expect 2-4 rounds of counteroffers over 4-8 weeks. Use your benchmarking data to justify the gap between their offer and your ask.
