Terms of Service
These terms govern your use of the ProEnrollment LLC website and our credentialing and payer enrollment services. By engaging ProEnrollment, you agree to these terms. Questions: info@proenrollment.com or (945) 307-6616.
Scope of Services
ProEnrollment LLC provides payor credentialing, payer enrollment, CAQH ProView management, Medicare PECOS enrollment, state Medicaid enrollment, re-credentialing management, and insurance contract negotiation. The specific services included in your engagement are defined in your written proposal. We prepare and submit applications on your behalf and follow up with payers; we do not make coverage, network participation, or reimbursement decisions — those belong solely to the payer.
Client Responsibilities
Accurate and timely information is essential to credentialing outcomes. You agree to provide complete, current, and truthful documentation (licenses, DEA registration, board certifications, malpractice coverage, work history) and to notify us promptly of any change to licensure status, practice address, tax identification, malpractice coverage, or disclosable adverse actions. Delays or denials caused by incomplete, inaccurate, or late-provided information are outside our control.
What We Do and Do Not Guarantee
We guarantee our process: applications audited before submission, submitted within the stated turnaround, followed up with every payer on a documented cadence, and deficiency responses within 24 hours. We do not and cannot guarantee payer approval, specific effective dates, panel openings, or reimbursement rates — these are determined by the payer, not by us. Published metrics such as our a 99.4% first-time approval rate describe historical performance and are not a warranty of future results for any individual application.
Fees and Payment
Fees are set out in your written proposal as a flat per-provider fee, monthly retainer, or new-practice package. Quoted pricing is valid for 30 days and is not subject to hidden surcharges, per-deficiency billing, or setup fees not disclosed at signature. Retainer engagements are month-to-month following the initial onboarding period. Fees cover our work and are not contingent on payer approval outcomes.
Confidentiality and Data Handling
All provider information is handled under a signed HIPAA Business Associate Agreement and the practices described in our Privacy Policy. Both parties agree to maintain the confidentiality of non-public information exchanged during the engagement.
Limitation of Liability and Governing Law
To the maximum extent permitted by law, ProEnrollment LLC's aggregate liability arising from the services is limited to the fees paid for the specific engagement giving rise to the claim. We are not liable for indirect or consequential damages, including lost revenue attributable to payer processing timelines outside our control.
