The Complete Provider Credentialing Checklist
Use this checklist to confirm every document and registration is in place before submitting payer applications. Missing or mismatched items here are the leading cause of credentialing delays.
Documents to Gather
- Active state medical/professional license (all states of practice)
- DEA registration (if prescribing)
- Board certification documentation
- Malpractice insurance face sheet showing coverage limits and retroactive date
- NPI confirmation — Type 1 (individual) and Type 2 (group, if applicable)
- CAQH ProView profile — complete and attested within 120 days
- W-9 and practice tax ID (EIN)
- CV with no unexplained employment gaps
- Medical school diploma and residency/fellowship completion letters
- Government-issued photo ID
Before You Submit
- NPPES record matches your application address exactly
- CAQH attestation is current (not expired)
- All license numbers verified active on the state board website
- Malpractice coverage meets each payer's minimum requirements
- Group NPI and tax ID match across all documents
