What Is CAQH ProView?

CAQH ProView is a centralized credentialing database used by 1,000+ health plans. When you apply to join any commercial payer network, they query your CAQH profile to verify your license, board certification, malpractice insurance, and work history. How to Set Up CAQH ProView

  1. Register at proview.caqh.org using your NPI number
  2. Complete all required sections: personal info, education, training, work history, licenses, DEA, board certifications, malpractice insurance, hospital privileges
  3. Upload supporting documents for all sections
  4. Authorize all target payers in the Authorizations section
  5. Complete attestation — must be renewed every 90–120 days

Section-by-Section Setup Walkthrough

Personal information: must match NPPES exactly — legal name, practice address, phone. Education and training: every program with month/year dates and completion documentation. Work history: five years minimum, no gaps over 30 days without written explanation, dates matching your CV. Licenses: every state, every type, with expiration dates current. Malpractice: carrier, policy number, limits, and effective dates with continuous coverage shown — upload the face sheet.
