CAQH ProView Setup Guide for New Providers
What Is CAQH ProView?
CAQH ProView is a centralized credentialing database used by 1,000+ health plans. When you apply to join any commercial payer network, they query your CAQH profile to verify your license, board certification, malpractice insurance, and work history. Without a complete, attested CAQH profile, no payer application can proceed. Learn about ProEnrollment's CAQH management service.
How to Set Up CAQH ProView
- Register at proview.caqh.org using your NPI number
- Complete all required sections: personal info, education, training, work history, licenses, DEA, board certifications, malpractice insurance, hospital privileges
- Upload supporting documents for all sections
- Authorize all target payers in the Authorizations section
- Complete attestation — must be renewed every 90–120 days
Most Common CAQH Mistakes
The top CAQH errors that cause credentialing denials: (1) NPI-NPPES address mismatch; (2) Wrong taxonomy code; (3) Expired attestation; (4) Missing payer authorizations; (5) Unexplained work history gaps. Free CAQH Readiness Tool | get started with ProEnrollment.
Section-by-Section Setup Walkthrough
Personal information: must match NPPES exactly — legal name, practice address, phone. Education and training: every program with month/year dates and completion documentation. Work history: five years minimum, no gaps over 30 days without written explanation, dates matching your CV. Licenses: every state, every type, with expiration dates current. Malpractice: carrier, policy number, limits, and effective dates with continuous coverage shown — upload the face sheet. Disclosure questions: answer honestly and explain anything flagged; payers verify against the NPDB anyway, and an unexplained yes is far better than a discovered omission.
The Authorization Step Everyone Misses
Completing your profile isn't enough — the Authorizations section controls which payers can access it. Either select global authorization or individually authorize every target payer; an unauthorized payer sees nothing and your application stalls with no error message anywhere. Then attest, and re-attest every 90–120 days forever: expired attestation freezes every in-flight application silently. Set calendar reminders or use managed attestation.
